Hair transplant recovery month by month

After a hair transplant, the new hair falls out before it grows back. Knowing the timeline in advance saves you a lot of worry.

After a hair transplant, small crusts form and fall off within about 10 days. Between weeks 2 and 6, most transplanted hairs shed; this is normal, because the roots stay in the scalp. New hair starts to grow around month 3 to 4, a clear change is usually visible around month 6, and the final result takes 12 to 18 months.

The timeline at a glance

WhenWhat usually happens
Days 1 to 3Swelling of the forehead is possible. The first wash is done at the clinic after 2 to 3 days.
Days 4 to 10Small crusts soften with daily washing and fall off. Redness fades.
Weeks 2 to 6Most transplanted hairs shed. The roots stay and rest.
Months 2 to 3The scalp looks much like before the procedure. Some people get small spots, like pimples, as new hairs come through.
Months 3 to 4New, fine hair starts to grow.
Months 6 to 9The hair becomes thicker and the change is clearly visible.
Months 12 to 18Final density and texture. The crown often takes longest.

The first two weeks

  • Sleep with your head slightly raised for the first nights.
  • Do not scratch or pick the crusts; let them come off with washing.
  • Avoid smoking and alcohol; they can slow healing.
  • Protect the scalp from direct sun and avoid swimming and saunas until your doctor allows it.
  • Light work is usually possible after a few days; heavy exercise waits longer.

The shedding phase is normal

Many patients worry when the new hairs fall out a few weeks after the procedure. This is called shock loss and is part of the normal cycle: the hair shaft falls, the root stays and starts a new growth phase. Existing hair around the area can also shed for a while and usually grows back.

When to contact the clinic

  • Increasing pain, redness or warmth after the first days.
  • Pus or a bad smell from the scalp.
  • Fever.
  • No visible new growth at all after about 6 months.

Keep the rest of your hair

A transplant does not stop hair loss in other areas. Ask your doctor whether medical treatment makes sense for you. Read FUE or DHI for the difference between the techniques and how to choose a hair transplant clinic if you are still comparing.
