Gynecomastia surgery: what men should know

Enlarged breasts in men are common and often have nothing to do with fitness. Here is how it is treated.

Gynecomastia means enlarged breast tissue in men. It can be caused by gland tissue, fat or both. Fat can respond to weight loss, but gland tissue does not. Surgery removes the gland and, if needed, the extra fat through small incisions, for a flatter chest.

Gland, fat or both?

Pseudo-gynecomastia is mainly fat and often improves with weight loss. True gynecomastia is a firm disc of gland tissue behind the nipple that stays even when you are lean. Most men have a mix of both. The surgeon checks this during the consultation.

Common causes

  • Hormonal changes during puberty; in many teenagers it disappears by itself.
  • Weight gain.
  • Some medicines and anabolic steroids.
  • Sometimes a medical condition, which is why a doctor should assess it first, especially if it appears suddenly or on one side.

How the surgery works

Through small incisions, often at the edge of the nipple, the surgeon removes the gland tissue. Liposuction shapes the chest and removes extra fat. If there is a lot of loose skin, for example after big weight loss, some skin may also need to be removed.

Recovery

WhenWhat usually happens
Day 1You usually go home or to the hotel the same day, wearing a compression vest.
Days 3 to 5Check-up; most men can walk around Istanbul comfortably.
Weeks 1 to 2Back to office work for most men.
Weeks 4 to 6The vest is worn day and night; light training can usually start again.
About 6 weeksChest training and heavy lifting are usually allowed again.
3 to 6 monthsSwelling has settled and the final chest contour is visible.

Will it come back?

Removed gland tissue does not grow back. Weight gain, steroids or certain medicines can enlarge the remaining tissue, so keeping a stable weight helps keep the result.
